1. Activities and risks
Basketball, volleyball, and other sports involve physical exertion, falls, collisions, and possible injury. Participants should use appropriate judgment and equipment.
2. Participant responsibility
Participants agree to follow facility safety rules, use equipment responsibly, and stop activity when unsafe. Medical emergencies should be reported immediately.
3. Medical assistance
A final agreement should explain emergency-contact procedures, any authorization for emergency medical assistance, and responsibility for associated expenses.
4. Parent or guardian consent
For participants under 18, a parent or legal guardian must review and complete any required consent. The final process must account for applicable Ohio law and age-appropriate supervision.
5. Legal release and acknowledgment
